MAKE A REFERRAL
Refer an NDIS Participant to NITPLANS Care Services

Thank you for choosing NITPLANS Care Services to support an NDIS participant.

We welcome referrals from participants, families, carers, support coordinators, plan managers, allied health professionals, hospitals, schools, and community organisations.

Our referral process is simple, secure, and usually takes less than 5 minutes.

WHO CAN MAKE A REFERRAL

  • NDIS Participants
  • Parents, family members, or informal carers
  • Support Coordinators
  • Plan Managers
  • Allied Health Professionals
  • Hospital discharge planners
  • Schools and transition providers
  • Community or government organisations

WHAT HAPPENS AFTER A REFERRAL

  1. Referral acknowledgement within one business day
  2. Initial contact with participant or nominee
  3. Service matching based on needs, funding, and preferences
  4. Service commencement (usually within 48–72 hours where possible)
